Booker Family Health Center, we see our highly trained and compassionate medical, nursing, and support team as a family committed to the health and well-being of yours.

Every day, our team works enthusiastically to help those in the community seeking our services. So whether your family has a sick child, an expectant mother, or an ailing grandparent, we'll be here for you and your loved ones because like you, we understand family is everything.


Comprehensive Care

The Jane H. Booker Family Health Center provides a wide range of acute and preventative health services to meet the needs of every member of your family.

As part of the Meridian Pediatric Network, our Center for Pediatrics has services that emphasize routine health screenings, parental education, and immunizations according to the progressive stages in a child's development. And for those times when your child is ill, our physicians and nurses are here to provide comfort and treatment.


Eligibility & Appointment Information

If you have Medicaid, Medicare, or any type of insurance coverage, please call 732-776-4209 for instructions on setting up an appointment. An appointment must be made in order to be seen in any of our service areas. If you are a member of a Health Maintenance Organization (HMO), your primary care physician must be a doctor with the Family Health Center for you to be eligible to use our services.

There is a charge for all medical services, however if you do not have insurance, unless you are pregnant, you must first complete a financial evaluation prior to seeing one of our doctors. Please call 732-776-4668 to schedule a financial interview, conducted Monday through Thursday in the Financial Assistance (Charity Care) Office. Bringing the following documents with you will greatly assist the process:

  • Proof of income (three months of pay stubs)
  • Proof of address (driver's license or envelope showing your current address)
  • Social Security card
  • Any type of insurance card
  • Checking and savings account statements

A complete evaluation of the financial evaluation and determination process will be provided during your interview. While in the process of applying for Charity Care, patients can schedule an appointment.  

Medicare & Charity Care Patients: The New Jersey Department of Human Services has instituted an insurance program called "Family Care." You may become part of an HMO. You will need to choose a primary care physician for yourself and your family. For more information on this important program, call 1-800-701-0710.
